The following forms need to be filled out by August 14th. Aside from the forms, physical photos and writing can be turned into Mr. King, Mrs. Bass, or a staff member, also due on August 14th. Physical forms are available in the Audio Visual room and the front office. Any questions can be directed to a previously mentioned person.

| Fill | Senior Information The yearbook staff will need the following information from you for the Senior section of the yearbook. Please fill out this form to the best of your ability. This form should be completed by August 14. Failure to do so may lead to your information being left out the book. Along with this form the yearbook staff needs a baby photo and a signature from each senior. The signature can be done when you turn in the photo to the yearbook room. This is also due on August 14.
